Thank you for your ski order.

In order for us to mount the bindings correctly and in line with industry guidelines, we require certain information.

Please Note: you need to complete a separate form for each pair of skis.

Please provide the following details in full (all fields are mandatory):

Order reference number
Name (on invoice)

Ski Model (Eg. Salomon BBR)
Age
Height (Feet & inches or cm)
Weight (Stones & pounds or Kg)
Ski Boot Make
Ski Boot Model
Ski Boot Size Mondo Point eg 26.0
Ski boot sole length (Usually marked on the heel area of the boot in millimetres)
Skier Type (See below)
Lockwoods will mount the skis and bindings using the information above. However as no actual boot has been provided we strongly recommend that they are checked before use by a qualified technician. Please tick the box to indicate that you have read this and confirm that you are happy for us to mount the bindings using the information provided.

Skier Type Descriptions

1- Beginner
Just starting off.

1 Intermediate
Ski conservatively. Prefer slower speeds. Ski on easy/moderate slopes - Blue and some Red runs.

2 Advanced
Skiers in good physical condition. Prefer a variety of speeds. Skis on varied terrain, including most difficult pistes.

3 Expert
Ski aggressively. Normally ski at high speed. Prefer moderate to steep terrain.

3+ Instructor/Racer
Very strong skiers, on challenging terrain.
